Our generation is the first to see opioids, mental health and substance use surpass chronic disease as leading causes of death in the United States.

Rising Disparities
The root causes of many health issues seen in our healthcare settings have nothing to do with physical health needs. Homelessness, mental health, and behavioral health are rooted in conditions outside of our hospital walls: our homes, family structures, and community systems.

Growing Community Needs
Local food banks, government-funded services, and healthcare systems are overburdened by growing community needs. Independently, we cannot solve the myriad of complex health and social needs. Through intentional collaboration and partnership, our community systems are the right solution for pressing and emerging community need.
